To request a duplicate of a lost or damaged report, please fill out the form below. Standard turnaround for duplicate requests is 1-2 business days. Associated fees will be billed upon delivery.

*Duplicates will only be issued if the original report is lost or damaged. Additionally, only one duplicate per report will be issued. If a report already has a duplicate, the client will need to resubmit the stone.
